Funeral Service for Chantel LaChanz Kemp-Simmons, 47 years of 5900 Townsend Road, Jacksonville, Florida and formerly of Nassau, who died at Doctors Hospital on Saturday, 28th September, 2019, will be held at New Providence Community Center, Blake Road on Saturday, 19th October, 2019 at 11:00 a.m. Pastor Jevon Neely will officiate. Cremation will follow.
We mourn her passing and will miss her smile, her love and support.
